Fees and Insurance
At any time please feel free to openly discuss our fees, your insurance
or your account with us. We participate in many insurance programs;
if you have any problems with your claims, we suggest that you deal
directly with your insurance carrier for best results. We will file
insurance claims for you. If there are any specific instructions or
questions, please let us know. Please be aware that some companies
pay a set fee for certain procedures and others pay a percentage of
the charge.
It is your responsibility to pay any deductible amount, co-insurance,
or any balance left unpaid by your insurance company. If your insurance
requires an office visit, co-payment must be made at the time of service.
It is our office policy that all patient responsible balances be paid
within 90 days of service.
Worker's Compensation Claims
As a courtesy to our patients, we will file worker's compensation
claims. However, if the claim is denied, unsettled or unpaid within
90 days, we will request that you file a personal health care claim
or pay the charges in full. If the situation becomes a legal matter,
you are still ultimately responsible for the payment of charges.
Record Copies
Your medical records are protected by federal and state law. All
medical records are confidential and will not be released without
your written permission. Your signature on the patient registration
form will allow us to release medical information to the health
care provider who referred you to this office, physicians, hospitals
involved in your care, and your insurance company.
Surgical First Assistants
Our practice utilizes a specially trained surgical nurse, Brenda
Banker, RN, RNFA, assisting the surgeons in the operating room.
As a provider of surgical assisting she may be reimbursed by your
insurance company for her services.
